This 13 episode anime (released in 2012) is about five high school students, Inaba Himeko, Nagase Iori, Yaegashi Taichi, Aoki Yoshifumi, and Kiriyama Yui. They are all members of a club at their high school called the Cultural Research Club A.K.A. the Student Cultural Society. Suddenly, a strange phenomenon occurs in which the students switch bodies amongst themselves. They face many different phenomenons, which are all controlled by a mysterious figure who goes by the name of Heartseed who demands to be entertained. Through each phenomenon, their friendship is put through ultimate test.
I watched this a few weeks ago, and it is already one of my favourite animes (although I haven't watched many myself. ^^;;). I'm not the expert on reviewing animes, but all I can say is: I recommend this anime if you are a fan of drama, romance, or slightly supernatural themes. ^^
Oh, man. I just watched this show, so I had to find the thread for it. Kokoro Connect was actually really great; the summaries on Crunchyroll really didn't do it justice. But either way, it's super awesome, and has likable characters.

Where did you hear that they were related? The animation studios, directors, and original work writers and publishes aren't the same either so I don't know what you're basing that on.
