Do you have their social media? Because you could just shoot them a message. If you don't have it, then try mutual friends. I don't think there's much else you can do.

If you get far enough that you're able to talk to them, then apologize first of all. How many years has it been since you guys fell out? If 5th grade was a long time ago then they've probably gotten over it already, but if not then don't push things if they don't want to talk to you. They might not even reply, but on your end, it'd probably help relieve some of that guilt to know that you tried reaching out.
