I like increased difficulty patches. I play a lot of strategy based games, and RPGs like Suikoden, FFT, and such. I start and stop them randomly. Insane Difficulty has some great patches for pretty much the best of the best PSx games if you're into emulating.

There's a mod for Left 4 Dead where the Tank becomes Shrek, and it's theme turns into Smashmouth.
